Regular eye exams protect more than your sight — they can reveal early signs of conditions such as diabetes and high blood pressure. Your UnitedHealthcare vision plan covers exams, lenses and frames, and you choose where to use it.

Using your benefit
How often you can use each benefit
  • Exam: once every 12 months
  • Lenses: once every 12 months
  • Frames: once every 24 months
  • Contact lenses: once every 12 months, in place of lenses and frames
Filing a claim
  1. In-network: there is no claim form. Give your Social Security number to your provider to access your benefits for payment.
  2. Out-of-network: complete the out-of-network claim form (opens in a new tab) and follow the instructions.
